SCHEDULING YOUR APPOINTMENT
PATIENTS: At your appointment, you will need to provide a copy of your recommending letter from a certified recommending doctor and/or your OMMCP patient registry card. When you arrive for your appointment, you will be asked to complete a brief medical history form, which will include a list of any medications you are currently taking.
